What the screening covers

Record symptoms, history and everyday impact.

The screening records current concerns alongside relevant history and examples from daily life. Information from different people or settings remains clearly labelled in the report.

1

Record what you see at home

Parent ratings and structured questions describe attention, activity, impulse control and the effect on daily family life.

2

Add school information if available

Optional teacher ratings remain separate, making agreement and differences between home and school visible in the report.

3

Describe the effect on daily life

Questions about learning, routines, friendships and family life provide practical context alongside the scores.

Questionnaires and scoring

See the screening tools and scores included.

The report names each tool and shows its score with the relevant published band or documented scoring rule. Personal history and non-normed questions are clearly labelled as context rather than clinical thresholds.

SNAP-IV parentA complete parent rating scale, shown with its symptom counts and descriptive results.
SNAP-IV teacherOptional school-setting evidence that remains visible without overriding the parent account.
VanderbiltAge-appropriate symptom and performance information, interpreted using the tool's documented rules.
Structured contextDevelopmental, education and day-to-day information that helps explain the scores.
