When an ADHD screening does not suggest ADHD

The most useful response to a below-threshold result is to compare it with the difficulties that led you to screen in the first place.

The short answer

A below-threshold result means the configured questionnaire did not find enough endorsed items for its screening threshold. It does not rule out ADHD, explain your difficulties or prevent you seeking clinical advice. Look at the complete report, including impairment, history, wellbeing and any missing information.

Start with what the score actually says

A screening threshold belongs to a particular questionnaire and version. Falling below it means your answers did not meet that rule. It is not the same as a specialist concluding that ADHD is absent.

NICE warns against making diagnosis from rating scales alone because both false positive and false negative results can occur.

Compare the result with daily life

Return to the reason you completed the screening. Are there persistent difficulties with attention, organisation, activity or impulsivity? Do they affect work, education, relationships or safety? Were they present in childhood? Does someone who knows you well see the same pattern?

If the answer is no, it may be reasonable to focus on a different explanation or practical support rather than pursue ADHD assessment.

Consider what the questionnaire did not capture

A short screener cannot examine every presentation. Missing informant information, uncertainty about childhood or a highly structured current environment may limit what can be inferred. Anxiety, depression, sleep problems, trauma and other health conditions may also need attention in their own right.

You can still ask for help

You do not need an above-threshold ADHD score to discuss concentration, organisation or emotional wellbeing with a GP. Take the report if it helps explain what you have noticed, including the below-threshold finding.

If symptoms are causing significant harm or distress, the need for help matters more than fitting one screening label.
